Please bring this completed form to your first visit.
Options (1) and (2) are the most common bowel preparation types. Most patients will use one of these two options. When your procedure is being booked, we will tell you which preparation to use for your colonoscopy.
Options (3) and (4) are reserved for patients who require enhanced bowel preparation. We will tell you if you need to use one of these options. Please check with our office if you are not sure which bowel preparation to use.
Follow these instructions if you are scheduled for a gastroscopy (upper gastrointestinal tract endoscopy).
Please follow these instructions for an outpatient operation (day surgery) such as a hernia repair, cholecystectomy, etc.
If you are coming for an endoscopic procedure, please use the instructions specific to your procedure.
